Centerpieces are objects or features places centrally within rooms that serve a variety of unique purposes. These Centerpieces are chosen randomly with each having a set chance to generate on a given floor and within a given type of room. When a room is generated, a pool of possible centerpieces is picked from, and one or none are chosen.

The Finger Altar and the Pugilism Altar share a model and texture but are different centerpieces. Cells marked "img needed" are placeholders: the imported table pointed every image at a file that was never uploaded.
Community notes on altars
- Finger Altar: costs a finger and offers a random item from a pool of weapons and bands. One player reports roughly a 1 in 11 chance for a specific band, and over 20 hours spent grinding for the last one. Patch 0.1.5 fixed a crash that happened when the altar had no items left to offer.
- Pugilism Altar: takes the weapon in your hand and drops the Bladed Ring in return. Patch 0.1.10 made it spawn more often.
- Boon Altar: one player's Kenneth Ring came from a boon altar.
- The Secret Rooms guide lists coin altars, boon altars and eater altars as the secret-room altars, alongside gravestones, weapon rocks and pots. It warns that eater altars cost maximum ring capacity but carry unique items; commenters push back on its low opinion of the coin and finger altars.
