The game has a Steam achievement for each ending, each boss, and several exploration and collection goals. The notes below are paraphrased from community guides and are not yet verified in-game.
Exploration and miscellaneous

| Ring Bearer | Wear a ring on every finger. One guide says 10 rings, but the ring guides list 8 finger slots (see Rings), so 8 is more likely. Whether sacrificing a finger at an altar blocks it is unknown. |
| Brain Devourer | Collect all nine concepts from enemy brains. |
| Underground Explorer | Visit the Underground. |
| Friendly Wizard | Talk to every NPC: the Warlock, the Vendor, the Knight, Peregrin, the Puppet, and the Demon (summoned with Summon Demon on a ritual circle in a secret room). |
| Golden Sacrifice | Offer gold (reported as 100) to the golden altar so the portal to the Gilded Halls opens. |

Endings
One achievement per ending. See Endings.
Collection
- Artefact Collector: collect every ring, band, eye, weapon, and the Camel. A guide suggests collecting all concepts and talking to every NPC (including the Mirror) to unlock most items, then buying them from shops or hoping for them at altars.
- Bestiary Keeper: record every regular enemy, boss and special enemy. Special enemies are The Face (from the Face Ring), Kenneth (from the Kenneth Ring), the Camels, and the Chaos Knight (which may appear after equipping a Chaos Ring).
- Blood Wizard: earn every other achievement.
